Pharmacology/ Pharmacokinetics:
Pharmacology-
Montelukast is a potent leukotriene modifier. It is widely used in treatment of Asthma
Fexofenadine hydrochlorode is a first generation anti-histamine . It is active carboxylic
acid metabolite of terfenadine . Unlike other H1- anti-histamine fexofenadine does not
cross the blood barrier readily thus overcome the common side effect ( drowsiness )
of its generation. It is indicated in the management of allergic rhinitis
Rationale for Combinaton-
Leukotrienes and histamines are potent mediators of bronchoconstriction and exert their
effect through cysteinyl leukotriene type- 1 and histamine receptors located in the bronchial
smooth muscle. The immunoglubulin E-dependent mast cell activation cascade causes
symptoms of allergic rhinitis marked by the release of inflammatory mediators including
histamine.
Patients with perrenial allergic rhinitis are also characterised by elevated levels of
cysteinyl leukotrienes in nasal lavage fluid. Hence histamine and cysteinyl leukotrienes
produce different responses in the pathogenesis of allergic rhinitis
Thus the concomittant administration of leukotriene antagonist and anti-histamine show
significant symptom relief and can be considered as effective therapy in allergic rhinitis
