Drug Interaction:
Concomittant use of oxytocin may lead to uterine rupture. Action enhanced by antiprogestins (epostane,mefpristone) When used for therapeutic abortion, oestrogen enhances cervical ripening and potentiates therapeutic effect of dinoprostone.
Caution admin of NSAIDs with prostagladins

Adverse Reaction:
Indication-
Cardigenic collapse due to accidental IV absorption at the time of intra-amniotic injection, uterine rupture,amniotic fluid embolism during labour. Foetal deaths due to uterine overstimulation. Occasional nausea, vomiting, diarrhoea, intra-partum foetal heart changes. Uterine contralile abnormalities with or without foetal distress.
Dosage
Exceeding 0.5mg intracervically can cause hypertonic uterine contractions.
I.V.admin: High incidence of nausea, vomiting, diarrhoea, abdominal pain, flushing, shivering, dizziness, hypotension, convulsions, local tissue irritation, erythmea.
Intra/Extra-aminotic inj: Local infection, vomiting, diarrhoea, pyrexia, transient hypotension.
Local gel: Rare. Vomiting, fever, diarrhoea, detectable myometrial activity more common with intravaginal appln rather than intercervical. Hyperstimualtion and foetal effects almost negligable.
Pessary: Vaginismus, severe pain,
GI disturbances, non recovery of pessary.
Contra-Indications:
Hypersenstive to prostagladin. Persons in whom oxytoxics are generally contraindicated. Ruptured membraine. Non-vertex presentation.
Special precautions:
Patients with glaucoma and raised introcular pressure, appln is endo-cervical and extra-amniotic, chronic pulmunary disease, previous ceseraian section.
Dosages/ Overdosage Etc:

Dosage:
Injection- 0.5ml endocervically at or near term pregnancy with poor Bishops score Suppositories- Insert one suppository(20mg) high into the vagina.
The patient should remain supine for 10 minutes follwing insertion. Administer each susequent suppository at 3 to 5 hour intervals until abortion occurs.
Patient Information:
1. Allergies- Tell your doctor if you have ever had any unusual or allergic reactions to dinoprostone other related medicines. Also tell your doctor if youn are allergic to any other substances such as foods, preservatives or dyes.
2. Other medicines- tell your doctor if you are taking any of the following- if you are using any other vaginal prescription or over the counter medicines
3.Other medical problems- tell your doctor if you have any other medical problems- Anemia - aborption with dinoprostone may result in loss of blood in some patients that may require transfusion Asthma or Ling disease - dinoprostone may cause bnarrowing of the blood vessels in the lung passages
4. Missed dose- If you miss a dose of this medicine take it as soon as possible. However if it is almost time for your next dose go back to your regular dosing schedule. Do not double doses.
