Avoid other that causes QT prolongation( eg quinidine, amiodarone, solatol, procainamide,
chlorpromazine, thioridazine, moxifloxacin, methadone )

May  potentiate antihypertensives, Alcohol, CNS depressants, Fluoxetine. paroxetine,
Clarithromycin, ketoconazole

Acute treatment of schizophrenia

Dizziness,    dry mouth,     fatigue,     nasal congestion,
ortostatic hypotension,     somnolence,   tachycardia,
increased weight,

QT prolongation ( discontinue if QTc > 500msec persists)
Priapism,    neuroleptic malignant syndrome,      tardive dyskinesia,
hyperglycemia,     dyslipedemia,    dysphagia,     hyperprolactinemia

Hepatic impairment not recommended
Lactation - not recommended

Special Precaution-
Bradycardia,  hypokalemia,   hypomagnesiumia,
congenital QT prolongation, recent MI ,  uncompensated heart failure
arrhythmias, avoid ( risk of Torsades de pointes /sudden death)
Cardio- or cerebral disease,

Monitor electrolyte especially K Mg

Diabetes or risk factors( obtain base line fasting blood sugar )
Monitor weight changes
History of breast cancer or eizures

Othostatic hypotension
Pre-existing Low WBC  count or leukopenia/ neutropenia

Monitor CBC during first few months of therapy
Discontinue if WBC decline

Exposure to extreme heat, dehydration
Suicidal  tendencies

Elderly( not dementia -related psychosis )
Noenates -risk of extrapyramidal and/or withdrawal symptoms  post delivery ( due to
exposure during third trimester of pregnancy )

Pregnancy

Indication-
Acute treatment of schizophrenia

Dosage-
1mg twice daily on Day 1
2mg twice daily on Day 2
4mg twice daily on Day 3
6mg twice daily on Day 4
8mg twice daily on Day 5
10mg twice daily on Day 6
12mg twice daily on Day 7

Target range - 6-12mg twice daily
Max- 24mg/day
Reduce dosage by half with concomittant CYP2D6/CYP3A4 Inhibitors
or poor metabolizers of CYP2D6

Retitrate if therapy is suspended > 3days
Reassess periodically
Patients below 18 years not recommended

Use contraindicated. Observe extreme caution if required to be used
